Prior to your appointment:

  • If you are an HMO member, obtain proper authorization (referral)
  • Any related previous medical records should be sent to the office.
  • A list of specific issues you want to discuss should be sent to the office.

At the time of your first appointment:

  • Referring doctor's name, address and telephone number.
  • Current insurance cards
  • List of all medications you are currently taking.
  • Download the following forms to fill out ahead of time (optional):
    • New Patient Form (not yet available online, please call for us to fax it to you)
    • Health History Form (not yet available online, please call for us to fax it to you)
