 Prior to your appointment:
- If you are an HMO member, obtain proper authorization (referral)

- Any related previous medical records should be sent to the office .

- A list of specific issues you want to discuss should be sent to the office.
|
At the time of your first appointment:
- Referring doctor's name, address and telephone number.
|

- List of all medications you are currently taking.
|
- Download the following forms to fill out ahead of time (optional):
- New Patient Form (not yet available online, please call for us to fax it to you)
- Health History Form (not yet available online, please call for us to fax it to you)
